Must be completed 2 weeks before event. List the title of the teleconference/webcast, program date, time, and specify if it's to be a "live audience" or "live audience and tape". You are responsible for program payment to the provider and forwarding of technical information to Wally Luksa, IDS-EVC. |

Telephony will deliver and set up the conference phone on the date you
request. It is your responsibility to return the conference phone to the
Help Desk, FOC 4 or to make arrangements to have the conference phone
picked up by calling (457-2900) or e-mailing the Help Desk. We will also make sure there is a working phone jack available. We also need to know if only outbound calls will be made or if outside parties will be calling in to the conference phone. It makes a difference on the kind of number we assign to the phone. |
